Registration:
Those who wish to attend the Congress should fill in the registration form. Please note that all persons intending to attend the Congress must register.

Print the registration form then either mail or fax the completed fom to the Congress Secretariat.

Full Registration and Student Registration includes:
n Welcome cocktail reception
n Access to all scientific sessions
n Access to trade exhibition
n Abstract submission rights
n Full congress materials including a copy of plenary, symposium lectures and abstracts.
n Certificate of participation
n Coffee/tea breaks, lunch daily


Student Registration:
Entitlement to a student registration requires that you do both of the following:

A) Submit an abstract for presentation (or be co-author of a paper)
B) Include a letter and proof of student status with your registration from your Head of Department stating clearly that you are a full-time student.

The full registration fee will be charged if this information is not provided.

PAYMENT OF FEES:
Registrations will not be processed or confirmed until payment in full is received.

Within Australia:
Payment may be made by personal cheque or credit card. Bankcard, Mastercard and Visa are the only credit cards accepted at the Congress. Please note, all transactions by credit card will appear on your statement as payment to 11th International Congress on Antiphospholipid Antibodies. Cheques should be made payable to: 11th International Congress on Antiphospholipid Antibodies.

International:
Payments from overseas must be made by credit card or bank cheque in Australian dollars only, drawn on an Australian bank. Your name and full address should be typed or printed clearly on the back of the cheque.

You may not pay your fees by direct transfer.

Acknowledgements:
Your registration and payment will be acknowledged in writing with confirmation of your requirements according to your registration form. Your registration will not be processed or confirmed if payment does not accompany your form.

You will not receive any further publications before the Congress. Your letter of acknowledgement will include any further advice necessary prior to your arrival at the Congress.

Letter of Invitation:
On request, the Organising Committee will send a letter of invitation to participate in the Congress. Such an invitation is extended specifically to assist participants to obtain travel funds or appropriate visas and does not imply any commitment on the part of the organiser to provide financial support. The letter will be forwarded to you with confirmation of your registration.

Registration and Information Desk:
The Registration Desk will be located in the Foyer of the Ballroom of the Sofitel Wentworth Sydney Hotel. The desk will be operating as follows:

Sunday 14 November        12.30 - 17.30
Monday 15 November        08.30 - 17.30
Tuesday 16 November       08.30 - 17.30
Wednesday 17 November  08.30 - 17.30
Thursday 18 November      08.30 - 16.00

Insurance:
Registration fees do not include insurance of any kind. It is strongly recommended that at the time you register for the Congress and book your travel you take out an insurance policy of your choice to cover any and every eventuality. The Congress Secretariat cannot take any responsibility for any participant failing to arrange their own insurance. This insurance is to be purchased in your country of origin.

CANCELLATION & REFUND POLICY:

Registration Fees:
Cancellation requests must be made to the Congress Secretariat in writing. For cancellations received by the 31st August 2004, a 30% administrative charge will be made. After that date refunds will not be possible but registration can be transferred to another person.

Accommodation:
Non-refundable as at 15 September, 2004.

Social:
Deposit is non-refundable as at 15 September, 2004.

Tours:
Tours are non-refundable as at 15 September, 2004.
